Community Physiotherapy and Occupational Therapy

Physiotherapy

The service aims to provide assessment, diagnosis and treatment for those with complex needs that are best met within their home environment.

Interventions include:

  • Developing a written strategy to help you manage your condition
  • falls assessments
  • information on how to prevent falls from happening
  • the provision of equipment
  • a risk assessment of your surroundings
  • exercise programmes
  • training and support of carers
  • provision of assistive technology and telemedicine to help the rehabilitation process

This service works closely with the Independent Living Team within each locality, as well as social services and the voluntary sector.

Occupational Therapy

This service is available for adults living in the North West of Lincolnshire. 

The service aims to provide assessment, diagnosis and treatment for those with complex needs that are best met within their home environment. 

Interventions include:

  • the provision of equipment
  • developing an alternative plan so you can continue to carry out your daily activities
  • hand splinting which gives protection and support for painful, swollen or weak joints

This service works closely with the Independent Living Team within the North West area of Lincolnshire, as well as social services and the voluntary sector.

The Occupational Therapy Service does not provide splints which actively assist movement.

Elements of the Service

The Community Physiotherapy and Occupational Therapy Service provides care for adult patients at home by the assessment and treatment of the following:

  • Parkinson's Disease
  • Strokes
  • Motor Neurone Disease
  • Multiple Sclerosis
  • Reduced mobility
  • Falls
  • Respiratory conditions
  • Osteoarthritis 
  • Rheumatoid arthritis

This list is not exclusive.

The service is also able to access support from other specialist services such as Palliative Care, Independent Living Team, Assisted Discharge Stroke Service, Musculoskeletal Physiotherapy Service and the countywide Respiratory Service.

Service Operating Times

 Monday to Friday from 8.30am to 4.30pm

Waiting Times for the Service

Waiting times vary depending on location and urgency of disorder.

Referral Details

Referrals are accepted from GP Practices, Community Nursing & Allied Healthcare Teams and hospitals.  Referral form or a letter of referral should be faxed or posted to the local service for your area.

Occupational Therapy will not accept referrals for the provision of equipment into nursing and residential homes and will not accept referrals from social services purely for equipment provision.
