Independent Living Teams

The aim of Independent Living Teams is to:

  • Promote faster recovery from illness
  • Prevent unnecessary hospital admission
  • Prevent admission to long-term care
  • Support discharge from hospital

The Independent Living Team is a joint service provided by Lincolnshire Community Health Service NHS Trust, and Lincolnshire County Council.

We are a team made up of Nurses, Physiotherapists, Occupational Therapists, Social Care Co-ordinators and Support Workers.

We provide short-term support, which may be a few days or weeks to help people remain in their own home safely, for example during a period of illness, or after a stay in hospital.  We may assist you with getting up, washed and dressed in the morning, preparing meals, or provide equipment such as a walking frame or commode.  

The aim of our service is to support you to do daily things – we are not here to do everything for you, but to help you to return to independence.

We offer our service mainly in your own home.  However, if you need support temporarily in a care home, we would arrange this and visit you there.

There is no charge for support provided by the Independent Living Team.

The Independent Living Team operates a 7 day service and works from 7am until 10pm.  Although the team will make every effort to offer times to suit you they CANNOT guarantee specific time slots for visits.

Waiting Times for the Service

The service responds to urgent needs within two hours, to non-urgent needs the same working day or as planned.

Referral Details

Referrals are accepted from professionals only (GP Practices, Hospitals, Community Nursing & Allied Healthcare Teams and Social Care) in order to ensure suitability for the service.
