P_CS_06 Admissions, Discharge and Transfer Policy for Community Hospitals

Lincolnshire Community Health Services NHS Trust (LCHS) has Community Hospitals across the county which provide pathways of care for patients with Rehabilitation, Urgent Care, Intermediate Care and End of Life Care needs.

The purpose of this policy is to establish safe and consistent pathways of care for patients with Rehabilitation, Urgent Care, Intermediate Care and End of Life Care needs.

The objective of this policy is to set clinical standards to improve the admission of appropriate patients and gives information about specific services at different sites.
