LCHS Child Health Records
LCHS Child Health Records Department is responsible for ensuring there is a health record on the system for every child aged 0-19 years, who is registered with a Lincolnshire GP, resident at a Lincolnshire County Council local authority address or schooled in a Lincolnshire Education Authority School.
The primary function of the service is to provide a comprehensive local record of a child’s health status ensuring each child has an accurate active record supporting delivery of public health interventions including:
- Screening
- Childhood and adolescent immunisations
- The Healthy Child Programme
