Specialist services are now planning for restoration and recovery.

During this period we continue to triage patients referred to us offering support via education leaflets, websites, telephone and digital consultations. Face to face home visits are available for essential visits only.

The service is provided by Respiratory Nurse Specialists, Case Managers and Complex Case Managers with a respiratory interest working in partnership with hospital staff, GPs, practice nurses and social care staff.

Description of the Service

The service is provided to patients with Chronic Obstructive Pulmonary Disease to facilitate early discharge from hospital with the community respiratory team providing specialist treatment and support at home in surroundings familiar to the patient.

Elements of the Service

The service provides full physical and social assessments for patients

We can provide the short term loan of nebulisers

Emergency short burst oxygen therapy can be provided if needed

We can prescribe treatment, as appropriate, to help with the recovery of an acute illness

Service Operating Times

The service is available from 9am to 5pm, Monday to Friday, excluding bank holidays.

Waiting Times for the Service

All patients are seen within 24 hours of being discharged from hospital unless it is a weekend where we will try and involve out of hours and community nurses if possible.
