Specialist services are now planning for restoration and recovery.

During this period we continue to triage patients referred to us offering support via education leaflets, websites, telephone and digital consultations. Face to face home visits are available for essential visits only.

The Heart Failure Team provides a specialist service to patients with unstable heart failure symptoms.

Description of Service

The aim of the service is to improve quality of life by managing patients' healthcare needs in their own homes. We also hope it will mean people can avoid having to make unnecessary hospital visits.

We work in partnership with Consultant Cardiologists, GPs and palliative care services to enable better care planning to support patients, their carers and family.

Elements of the Service

  • Education and support to increase patients' skills and confidence in managing their own health problems including regular assessment of progress and problems, goal setting, and problem-solving support
  • Working with other professionals we review the most complex heart failure patients and those with palliative care needs
  • We carry out assessments and develop treatment plans with patients and their carers and families and other professionals
  • We support end of life decisions and care

The service is available from 9am to 4pm, Monday to Friday.

Waiting Times for the Service

Patients are seen within 10 days of referral or discharge from hospital. For existing patients, with any significant change to treatment or condition, contact and assessment will be prioritised according to need.

Referral Details

Referrals are accepted from GPs, Community Hospitals and community healthcare professionals.
