Urgent Community Response
Sometimes patients may need urgent support to be able to continue stay at home and avoid going to a hospital. Urgent Community Response can be called and within two hours to assess and identify care and support services to enable patients to remain safe and well in the comfort of their own home. In many cases, patients can avoid numerous and sometimes distressing trips to A&E.
We believe that people are recovering better when they at home or their place of residence. Staying at home allows them to be more independent and live healthy lives for as long as possible.
Support for patients may be delivered by community care services, adult social care services, care homes, home care services and voluntary organisations.
Urgent community access and referral process
Patients can be referred by any health, care and social care professional.
The service operates seven days a week 8am to 8pm with referral line open until 6.30pm every day.
